This form is used to evaluate the ongoing service of any Big Dawg,  Academic Lab Tutor, Office or Dean Aide, Team Manager, H.A.W.C. Member, P.A.C. Member, Religious Education Aide, Teen Advisory Council Member, etc. It is the student's responsibility to provide his or her supervisor with a copy of the form with his or her name printed on it. Likewise, it is the student's responsibility to retrieve the form from his or her supervisor and turn it in to the NHS Adviser. Evaluations will be collected in September and January for use in National Honor Society member growth conferences.
