Contact Revision Form

Grant is excited to re‐introduce the adoption of the ALERTNOW rapid Notification
Service system as a tool to communicate school delays or cancellations due to
inclement weather or emergencies. The program was originally launched during
the 2007‐2008 school year and received a positive reaction from the community.
It works like this: via an automated dialing system, parents and guardians of our
students will receive a telephone call or e‐mail message with notification of a
delayed start or closure. ALERTNOW is a web based service able to reach
thousands of contacts at rates up to 6,000 calls per minute and all without using
the school’s phone lines.

In order for the ALERTNOW system to effectively serve our parents and students,
it is vital that the contact information in student records be up‐to‐date. Parents
can help keep communication lines open and accurate by providing the
appropriate information for their student’s files. For your convenience here is a
Contact Revision Form so that you may help update our records. Please keep in
mind that Alert Now will first contact the primary phone number on file as well as
one e‐mail address. Should there not be a voicemail or answering machine
available at the primary phone number it will go to the next phone number on
file. The program gathers the contact information from the Personal Information
Form provided by families to the school at registration time. The Contact Revision
Form should only be submitted if there are changes to the telephone contact
information and/or e‐mail address provided the school for the most recent school
year. Please make current a Primary Phone Number and up to two alternates and one e‐mail
